Our website now has a member-only section where you can browse and select health promotion messages and patient experience surveys within our library, support your QI projects or register in a study.

How does it work? Is there a cost to become a member?

There is no cost to become a member. Providers can browse our library of messages and surveys for free, or learn more about and register for our studies. If you would like to send a message or a survey to your patients, you have several options including distribution through your own portal or via CPIN. Distribution and data collection may be free or may include a small cost-recovery fee, depending on level of assistance requested. Participation in studies is always free.

As a primary care provider or clinic manager, you can create a user account by filling out the form below. You will be asked a few additional questions that will be saved onto your account making your future experiences using the website more seamless.
